First you need a item...
To make it +6 you need 6 Bless
and to make it +9 you need 3 souls
of course you have to add the 6 Bless first and then the 3 souls.

Go to Chaos goblin that you can find in Noria [180,100]
Choose Option "Regular Combination"
First you need:
#Item+9
#1 Chaos
#1 Soul
#1 Bless

NOTE: The combination can fail of course...

Now choose again Option "Regular Combination"
2nd you Need:
#Item+10
#1 Chaos
#2 Souls
#2 Bless
NOTE: The combination can fail of course...

Now choose again Option "Regular Combination"
3rd you Need:
#Item+11
#1 Chaos
#3 Souls
#3 Bless

Now choose again Option "Regular Combination"
4th you Need:
